- Portfolio should be 1 document of presentation (DO NOT SEND SEVERAL ATTACHMENTS IN AN EMAIL)

- Items that should be in the portfolio include:
- Cover Page (Must represent you and personal fitness)
- Table of Contents
- Inspirational page #1(Inspirational image, quote or saying, and short passage explaining why it inspires you)
- Writing prompt #1: Self-image, goals, and plan
- Fitnessgram data (You can google this information)
- Fitness log sheet (Make sure to type your summary. Compare where you started to where you are now. It should be about 1 paragraph)
- Exercise contract
- Inspirational page #2 (Inspirational image, quote, and short passage explaining why it inspires you)
- Writing prompt #2: Progress
- Meal planner/Record of 1 full week. 1 full planned out week of a healthy diet and 1 full week of what you actually had to eat.
- Calories should make sense
- Record liquids also
- Record everything that you eat
- Plan every meal (You cannot plan not to eat)
- Progress tracker (4 weeks/1 month, which is one page)
- Physical activity plan (4 weeks/1 month, which is one page)
- Inspirational page #3 (Inspirational image, quote, and short passage explaining why it inspires you)
