Portfolio


Portfolio Information

 

  • Portfolios are due on Friday 12/16/16
  • Portfolio must be submitted in an electronic format
  • Portfolio should be 1 document of presentation (DO NOT SEND SEVERAL ATTACHMENTS IN AN EMAIL)
  • Email portfolios to coachrogers.health@hotmail.com
  • Items that should be in the portfolio include:
    • Cover Page (Must represent you and personal fitness)
    • Table of Contents
    • Inspirational page #1(Inspirational image, quote, and short passage explaining why it inspires you)
    • Writing prompt #1: Self-image, goals, and plan
    • Fitnessgram data (You can google this information)
    • Fitness log sheet (Make sure to type your summary. Compare where you started to where you are now. It should be about 1 paragraph)
    • Exercise contract
    • Inspirational page #2 (Inspirational image, quote, and short passage explaining why it inspires you)
    • Writing prompt #2: Progress
    • Meal planner/Record of 1 full week. 1 full planned out week of a healthy diet and 1 full week of what you actually had to eat.
      • Calories should make sense
      • Record liquids also
      • Record everything that you eat
      • Plan every meal (You cannot plan not to eat)
    • Progress tracker (4 weeks/1 month, which is one page)
    • Physical activity plan (4 weeks/1 month, which is one page)
    • Inspirational page #3 (Inspirational image, quote, and short passage explaining why it inspires you)
    • Writing prompt #3 Final Thought

Week of 12/5/15 - 12/9/16




Monday - Students developed short in-class projects covering different sections from chapter 4, Nutrition and your Personal Fitness. Each group developed a mini lesson and a game to review their lesson. Students will start presenting their information. Students will participate in a student led workout for the second half of class. Workout will include warm-up, workout, and cool-down led by peers.

Tuesday - Students will work on final exam study guide. Students will participate in workout the 2nd half of class.

Week of 11/7/16 - 11/11/16

Monday - Students will start chapter 3 using guided notes, PowerPoint and open discussion. Once students complete lesson 1, they will go complete a workout on the track. Students will run around the stadium and up the stairs. Students will then complete an ab workout, and end with stretching.


Tuesday - Students will work through chapter 3 lesson 2. Students will use information given to them to compare their pre-assessments measurements to standards. They will write how they will make progress to their fitness levels by changing their FITT. Students will participate in a workout for the 2nd half of class. The workout will be on the field. Students will do push-ups, sit-ups, lounges, squats, and sprints. They will participate in an ab workout then end with stretching.


Wednesday - Students will complete chapter 3 lesson 3 using PowerPoint, guided notes, and open discussion. Students will participate in a class workout for the 2nd half of class. The workout will consist of students running a mile. The mile will be untimed. It will prepare them for their final mile of the class.


Thursday - Students will finish chapter 3. Students will complete the diet part of their personal fitness program that they are developing. They will participate in a workout the 2nd half of class. The workout will consist of students running hills outside of the health classroom. Students will do an ab workout after the hill and end with stretching.


Friday - Students will start their active challenge for their school. The 2nd half of class they will participate in a cardio workout. The cardio workout will be students running around the track.

Week of 10/31/16 - 11/4/16

Monday - Students will take the test for chapter 1. Students will start chapter 2 vocabulary. Then students will start chapter 2 using guided notes, PowerPoint and open discussion. Week of 10/17/16 - 10/21/16 Welcome to Personal Fitness

Monday - Syllabus, class procedures. Tour of facilities that will be used. Students will take their height, weight pre-assessment measures.
Tuesday - Students will take the written part of their common assessment. Students will develop their daily warm-ups then students will complete their pre-assessment data, sit-n-reach, push-ups, and sit-ups.
Wednesday -  Students will complete their Exercise Contracts. After contracts students will run their 1 mile as their pre-assessment.
Thursday - Students will complete vocabulary for chapter 1 and will start notes for chapter 1 and 2. Students will continue their pre-assessment data. Students will participate in a workout.
Friday - Students will finish chapter one notes then complete the pre-assessment for the pacer running test.

Week of 10/10/16 - 10/14/16: Final Week of Health

Monday - Students will start chapter 27 CPR and First aid. Students will complete chapter 27 vocabulary and go through chapter 27.1
Tuesday - Students will continue their lesson on CPR and first aid. Students will practice their CPR skills in class. Students will complete notes and practice CPR skills.
Wednesday - Students will complete CPR lesson. Students will practice CPR skills, what to do when someone is choking, and how to address wounds.
Thursday - Students will take final exam for Health. Discuss procedures for Personal Fitness, which starts Monday. Students need to bring dress out clothes for Personal Fitness. Dress out clothes is gym clothes or something that they can workout in.
Friday - Students will have a sub. Students will watch a video and answer questions following the video.

Week of 10/3/16 - 10/7/16: Alcohol, Illegal Drugs, and STDs


Monday - Students will start chapter 21 active notes. Chapter 21 covers alcohol.


Tuesday - Students will complete chapter 21 notes. Following notes students will develop a 7 question survey that they will ask their peers concerning alcohol use in their community. Students will ask 5 of their peers in the classroom and develop a summary from their questions and answers.


Wednesday – Students will start chapter 22, which covers illegal drugs. Students will complete vocabulary for the chapter then they will start the notes. Students will be given the notes to follow during open discussion. Students will complete the chapter.


Thursday – Students will cover chapter 24 STDs. Students will be given the notes for chapter 24 also, so they can follow along with open discussion. Following the chapters, students will complete an assignment that incorporates all three chapters. The assignment will help students understand how drug use can lead to situations which put them at risk for STDs.


Friday – Unit assessment. Students will be given an assessment that covers the 3 chapters.

Monday - Finish chapter 20 Tobacco. Students will work in small groups and present their short presentations on why someone should stay away from tobacco.

Tuesday - Students will take their unit assessment over chapter 21. Students will complete vocabulary for chapter 21 Alcohol and will be given the PowerPoint that covers alcohol. Students will use their handout to cover the chapter using PowerPoint and open discussion.

Wednesday - Students will present their health projects to the class. Students must have a 4 page paper emailed to the teacher before class on Tuesday and they must have their presentation ready that covers what was written in their papers.

Week of 9/12/16 - 9/16/16 Tobacco and Health Projects

Monday - Students will continue their work on their health projects. 1st and 2nd Block will go to the  media center and 3rd block will work on laptops in  the classroom. This may carry over to Tuesday if needed.
Tuesday - Students will start chapter 20 lesson 1 notes using PowerPoint. Students will write a letter to a friend or family who has begun using tobacco. Student's letters must be written correctly; therefore, students cannot use slang, acronyms, nor can they write like they speak.
Wednesday - Students will complete chapter 20 lesson 2 notes. Students will create posters that shows the harmful substances in tobacco products in small groups. A poster might have an illustration of a burning cigarette with callouts that identify and explain nicotine, carcinogens, tar, and carbon monoxide.
Thursday - Students and teacher will cover chapter 20 lesson 3 filling in their notes as the teacher explains the dangers to using tobacco. Students will complete their tobacco posters.
Friday - Students and teacher will complete notes for chapter 20. Cooperative Learning: Students will start working on Bulleting Boards about Smoking. Students will be given cooperative learning cards.

Week of 9/6/16 - 9/10/16: Health Projects

Monday - Students will start chapter 20 active notes. Chapter 21 covers Tobacco.
Tuesday - Students will continue chapter 20 notes. Students will start work on health projects. Groups for their health projects will be developed. Students will be given a rubric and expectations for their projects. Projects will consist of a 4 page paper and a presentations that outlines their paper. Each group will turn in a paper and present their presentation to their peers. Once groups are developed students will create an outline for their papers, each group member will understand his/her responsibilities, and the group will decide on the kind of presentation that the group will like to develop. All of this should be complete before the computers are delivered for the next three days so students can maximize their computer time.
Wednesday - Students will work on health project for the entire class block. Students will work on their 4 page paper and the presentation that goes along with their paper. Students will have rubrics to guide them through the project.

Week of 8/29/16 - 9/2/16: Reproduction and Endocrine Health

Monday - The teacher and students will complete the notes for chapter 16.1 and 16.2 using PowerPoint and open discussion. Students will start their group assignment, which is to develop a learning pamphlet for middle school students using visuals and facts about the reproduction system.

Tuesday - Students will complete chapter 16 and 17.1 using PowerPoint and open discussion.

Wednesday - Students will review chapter 16 and lesson 1 of chapter 17 to get ready for their test with a cooperative learning assignment. Students will get into small groups and will be given a topic from chapter 16 or 17.1. They will develop a short lesson plan to teach it to their peers. They will also develop a review game.

Thursday - Students will present their lesson that they developed and the class will participate in their review games.

Friday -  Students will watch "The Miracle of Life" video. Follow the video students will have their 2nd writing prompt.

Week of 8/22/16 - 8/26/16: Reproduction and the Endocrine System

Monday - Student will complete writing prompt #1 and they will have their first notebook check. 

Tuesday - Students will complete cooperative learning activity.

Wednesday - Students will take chapter 5 unit assessment. Students will complete vocabulary for chapter 16 and 17. The next unit will be Reproduction. Students will be given handouts of the reproduction system to label then students and the teacher will cover chapter16 lessons 1 and 2. As the teacher covers the lesson students will be able to check their labeling and make corrections.
