Portfolio Items

  • You will lose 10 points for each day they are late and I will not accept them after Monday!
  • Portfolios are due on Friday 12/15/17
  • Portfolio must be submitted in an electronic format
  • Portfolio should be 1 document of presentation (DO NOT SEND SEVERAL ATTACHMENTS IN AN EMAIL)

  • Items that should be in the portfolio include:
    • Cover Page (Must represent you and personal fitness)
    • Table of Contents
    • Inspirational page #1(Inspirational image, quote or saying, and short passage explaining why it inspires you)
    • Writing prompt #1: Self-image, goals, and plan
    • Fitnessgram data (You can google this information)
    • Fitness log sheet (Make sure to type your summary. Compare where you started to where you are now. It should be about 1 paragraph)
    • Exercise contract
    • Inspirational page #2 (Inspirational image, quote, and short passage explaining why it inspires you)
    • Writing prompt #2: Progress
    • Meal planner/Record of 1 full week. 1 full planned out week of a healthy diet and 1 full week of what you actually had to eat.
      • Calories should make sense
      • Record liquids also
      • Record everything that you eat
      • Plan every meal (You cannot plan not to eat)
    • Progress tracker (4 weeks/1 month, which is one page)
    • Physical activity plan (4 weeks/1 month, which is one page)
    • Inspirational page #3 (Inspirational image, quote, and short passage explaining why it inspires you)

Week of 11/6/17 - 11/10/17

All portfolio worksheets can be found under "worksheet" to the right

Monday - Students will work through chapter 3 lesson 1. Students will use information given to them to compare their pre-assessments measurements to standards. They will write how they will make progress to their fitness levels by changing their FITT. Students will participate in a workout for the 2nd half of class. The workout will be on the field. Students will do push-ups, sit-ups, lounges, squats, and sprints. They will participate in an ab workout then end with stretching.


Tuesday - Students will take chapter 2 assessment. Students will complete chapter 3 lesson 2 using PowerPoint, guided notes, and open discussion. Students will participate in a class workout for the 2nd half of class. The workout will consist of students running a mile. The mile will be untimed. It will prepare them for their final mile of the class.


Wednesday - Students will finish chapter 3. Students will complete the diet part of their personal fitness program that they are developing. They will participate in a workout the 2nd half of class. The workout will consist of students running hills outside of the health classroom. Students will do an ab workout after the hill and end with stretching.


Thursday - Students will start their active challenge for their school. The 2nd half of class they will participate in a cardio workout. The cardio workout will be students running around the track.


Friday - Students will complete writing prompt number 2. There will be a sub so students will not dress out for workouts.

Personal Fitness Week of 10/16/17

Monday - Syllabus, class procedures. Tour of facilities that will be used. Students will take their height, weight pre-assessment measures.
Tuesday - Students will take the written part of their common assessment. Students will develop their daily warm-ups then students will complete their pre-assessment data, sit-n-reach, push-ups, and sit-ups.
Wednesday -  Students will complete their Exercise Contracts. After contracts students will run their 1 mile as their pre-assessment.
Thursday - Students will complete vocabulary for chapter 1 and will start notes for chapter 1 and 2. Students will continue their pre-assessment data. Students will participate in a workout.
Friday - Students will finish chapter one notes then complete the pre-assessment for the pacer running test

Week of 9/18/17 - 9/22/17

Monday - Students will continue their work on their tobacco commercials.
Tuesday - Students will present their commercials to the class.
Wednesday - Students will start chapter 21 notes. Students will create posters that shows the harmful effects of alcohol in small groups.
Thursday - Students and teacher will finish chapter 21 filling in their notes as the teacher explains the dangers to using alcohol. Students will complete their alcohol posters.
Friday - Students and teacher will complete notes for chapter 22. Cooperative Learning: Students will start working on Bulleting Boards about tobacco, alcohol, or illegal drugs. Students will be given cooperative learning cards.

Week of 9/11/17 - 9/15/17 Tobacco

Monday - Students will continue start chapter 20 tobacco. Students will complete chapter 20.1 notes and develop groups for their performance assignment.

Tuesday - Students will continue chapter 20 lesson 2 notes using PowerPoint. Students will write a letter to a friend or family who has begun using tobacco. Student's letters must be written correctly; therefore, students cannot use slang, acronyms, nor can they write like they speak.

Wednesday - Students will complete chapter 20 lesson 3 notes. Students will create posters that shows the harmful substances in tobacco products in small groups. A poster might have an illustration of a burning cigarette with callouts that identify and explain nicotine, carcinogens, tar, and carbon monoxide.

Thursday - Students and teacher will cover chapter 21 lesson 1 and lesson 2 filling in their notes as the teacher explains the dangers to using tobacco.

Friday - Students and teacher will complete notes for chapter 21. Cooperative Learning: Students will work from choosing the best starting on page 11.

Week of 9/4/17 - 9/8/17

Monday - No School

Tuesday - Students were given a topic from chapter 16 and 17. They developed a lesson plan for their peers and a review game to cover their material. Students will present their lesson that they developed and the class will participate in their review games.

Wednesday -  Students will watch "The Miracle of Life" video. Follow the video students will have their 2nd writing prompt.

Thursday - Students will take their unit 16 and 17 assessment. Following the assessment students will complete vocabulary for chapter 20 Tobacco.

Friday - Students and teacher will cover chapter 20 lesson 1 and 2 using PowerPoint and open discussion.

Week of 8/21/17 - 8/25/17 Reproduction and Endocrine Health

Monday - Student will complete writing prompt #1 and they will have their first notebook check.

Tuesday - Students will complete cooperative learning activity.

Wednesday - Students will take chapter 5 unit assessment. Students will complete vocabulary for chapter 16 and 17. The next unit will be Reproduction. Students will be given handouts of the reproduction system to label then students and the teacher will cover chapter16 lessons 1 and 2. As the teacher covers the lesson students will be able to check their labeling and make corrections.

Thursday- The teacher and students will complete the notes of chapter 16 using PowerPoint and open discussion. Students will start their group assignment, which is to develop a learning pamphlet for middle school students using visuals and facts about the reproduction system.

Friday - Students will review chapter 16 and lesson 1 of chapter 17 to get ready for their test. Students will watch "The Miracle of Life" video. Follow the video students will have their 2nd writing prompt.

Week of August 14 - 18: Mental and Emotional Health

Monday - Students start vocabulary for chapter 5 Mental and Emotional Health

Tuesday - The teachers will cover chapter 5 lesson 1 and 2 using PowerPoint and open discussion. Students will complete a Cooperative Learning assignment. This will be group work.

Wednesday - Teacher and students will complete chapter 5 lesson 3 using PowerPoint and open discussion. Students will watch short videos describing different mental disabilities. Students will develop Anti-Suicide material. They will make a flyer, brochure, or poster that delivers a positive message and provide accurate information for students who may be thinking about suicide.

Thursday - The class will complete chapter 5 notes using PowerPoint and open discussion. Students will start their Eye on the Media assignment. Students will develop a brochure, billboard, a newspaper add, and a slogan encouraging people to see help for mental and emotional problems. The will be a group assignment that will take two days to complete.

Friday - Students will complete their Eye on the Media assignment. Students will watch short film covering Mental and Emotional Health. Students will have a writing assignment once the film is over.

Final Weeks of Personal Fitness


Portfolio Information

Writing Prompt #1: Overview: My Self-Image
Directions: Writing prompt must be at least 1 page long. Remember how we write. Refer back to your writing rubric. Be specific, give examples and answer each question in complete sentences.


Topic: What is your current self-image (How do you feel about yourself)? What would you like to change about yourself physically, mentally, and/or emotionally? If you would not change anything please explain why. What are your goals for personal fitness? How do you plan to reach your goals?


Writing Prompt #2: Mid-Semester Progress
Directions: Writing prompt must be at least 1 page long. Remember how we write. Refer back to your writing rubric. Be specific, give examples and answer each question in complete sentences. Topic: How do you feel concerning your progress in personal fitness thus far? What progress have you made or have you not made any progress? Please explain why you feel the way that  you do concerning your progress. What needs to change for you to reach your goals?

Final 3 Weeks of Health 2/27/17 - 3/17/17

Week of 2/27/17 - 3/3/17


Monday - Students will start chapter 21 active notes. Chapter 21 covers alcohol


Tuesday - Students will complete chapter 21 notes. Following notes students will develop a 7 question survey that they will ask their peers concerning alcohol use in their community. Students will ask 5 of their peers in the classroom and develop a summary from their questions and answers.


Wednesday – Students will start chapter 22, which covers illegal drugs. Students will complete vocabulary for the chapter then they will start the notes. Students will be given the notes to follow during open discussion. Students will complete the chapter.


Thursday – Students will cover chapter 24 STDs. Students will be given the notes for chapter 24 also, so they can follow along with open discussion. Following the chapters, students will complete an assignment that incorporates all three chapters. The assignment will help students understand how drug use can lead to situations which put them at risk for STDs.


 Friday – Unit assessment. Students will be given an assessment that covers the 3 chapters.

Week of 3/6/17 - 3/10/17: CPR and First Aid


Monday - Students will start chapter 27 CPR and First aid. Students will complete chapter 27 vocabulary and go through chapter 27.1

Tuesday - Students will continue their lesson on CPR and first aid. Students will practice their CPR skills in class. Students will complete notes and practice CPR skills.

Wednesday - Students will complete CPR lesson. Students will practice CPR skills, what to do when someone is choking, and how to address wounds.

Thursday – Students will participate in their CPR check-off and written exam. Students who pass both the check-off and the written exam will receive their CPR certification cards.  

Friday – Final exam study guide.

Week of 3/13/17 - 3/17/17: Alcohol and Drug Awareness Program (ADAP)


Monday - Students will cover chapter 1 and 2 of the ADAP manual. ADAP is required for students to get their divers licenses.


Tuesday - Students will cover chapters 3 and 4 of ADAP using open discussion and PowerPoint. Students will be given worksheets to help them review the information.


Wednesday - Students will review their ADAP information and take the written exam. Students must pass the exam with a 70 in order to get their certificate. Students must be present in class all 3 days to receive their certification.


Thursday - Final exam for health.


Friday - Transition to Personal Fitness
